Rehabilitation Sessions
Momentum provides rehabilitation sessions in your home on a regular or occasional basis. Sessions can be 1-3 hours in length - depending on your goals.
Sessions are tailored to each individual, based on your needs and your goals. They are specifically designed to incorporate multiple areas of focus to promote an active lifestyle and promote movement, problem solving and autonomy.
Specific skill based sessions are also available. These can include any activity in your home that you wish to become more independent with. Do you want to cook independently? What about laundry? More independent transfers? Activities of daily living?
Perhaps you would like to learn how to incorporate what you are doing with us in sessions into your daily life and learn how to apply newly learned skills in your own environment. There are so many possibilities…

Support in your Community
As an extension to our skill based sessions we also offer supports in the community.
Perhaps you have a individual goal in mind and it requires specific support.
Maybe your child would benefit from a daycare/preschool/school consult.
Maybe you need some workplace suggestions or modifications to increase independence and productivity.
Perhaps you want to go to a relatives home or restaurant and would like support learning how to access the building in the safest way.
Or imagine having support to show your PSW/caregiver how to best support you while maximizing opportunities for independent movement.
The possibilities are endless…
